We are reaching out to share an important update regarding our practice and your ongoing care.
Dr. Bender will transition out of our practice effective August 5, 2026. We are grateful for the care and dedication Dr. Bender has provided to our patients, and we wish her the very best in her next chapter.
Additionally, Dr. Verdiales will be taking a temporary leave of absence from the practice beginning August 17, 2026 due to unforeseen circumstances. We estimate her to return in 2 months. We ask that you respect Dr. Verdiales’ privacy during this time and appreciate your understanding.
We recognize that changes to your care team may raise questions or concerns. Our priority is to ensure continuity of care and to assist patients in making appropriate arrangements for their ongoing healthcare needs during this transition. Patients with upcoming appointments, active treatment plans, pending referrals, or other ongoing clinical needs will be contacted individually regarding any changes to their care, scheduling, or other necessary arrangements.
Establishing Care With Another Provider
If you have ongoing medical needs or an active treatment plan, we encourage you to establish care with another healthcare provider as soon as reasonably possible. You may choose a provider of your preference, and we encourage you to schedule an appointment promptly if continued medical care is needed.
Our office can assist with the transfer of your medical records to a provider of your choice. To request your records, please contact our office and complete the appropriate Authorization for Release of Medical Records form. You may request that a copy of your records be provided directly to you or transferred to your new healthcare provider. Please allow our team 72 hours to complete medical record release requests.
Please note that requesting your medical records does not establish care with another provider. You are responsible for selecting a new healthcare provider and scheduling an appointment with that provider.
If you have an upcoming appointment, pending treatment, prescription-related matter, referral, or other active clinical need, please contact our office at 770-266-0933 so that our administrative team can assist with the appropriate transition arrangements.
Practice Operations During This Transition
During this transition, we will continue to operate as a supplement store.
Our administrative team remains available to assist with scheduling matters, medical records, referrals, and other non-clinical questions. Our administrative team is not able to provide medical advice, diagnose conditions, prescribe medications, or make clinical treatment decisions. For urgent or emergency medical concerns, please seek appropriate medical attention without delay.
We are committed to handling patient records appropriately and assisting patients with the transition of their care. We encourage you to request your medical records and establish care with another healthcare provider if you require ongoing medical care.
